Auto Accidents Form Any automobile accident claims will need the following information from your insurance company. 1. Claim Number 2. Adjuster's Name and Number 3. Police Report (if possible, not necessary) 4. Copy of Insurance Card or all information.
Workers Comp Form (All patients that are claiming a w/c injury must have already filed an injury report with their employer. You should have a copy of your injury report to bring with you here. We need the worker's compensation insurance company information that your company maintains. We also need a claim number or reference number for you claim. If you do not have this, please make a prior call to your company and get this information.)
All patients using insurance of any kind will need to bring in their insurance card for verification purposes. It would be greatly appreciated if you could call your insurance company first to find out your coverage and benefits plan for chiropractic care.
Medicare and Denali Kid Care Patients need not call their insurance company. We do still need proof of current insurance in the means of the identification card.
